SECURING COPY OF ACCOUNTANT’S ADVICE OF CHECK DISBURSEMENTS
An Accountant’s Advice of Check Disbursement is a chronological list of checks issued for the day by the Municipal Treasurer’s Office verified by the Municipal Accountant and submitted to the LGU Depository Bank for their information and reference. This can be used by any person or business entities who have been paid by the LGU of goods purchased to them and services rendered as support for their check encashment at the LGU depository bank or to any bank of their preference.

- Register in the logbook Request for a Copy of Accountant’s Advice
- Personnel Incharge will scan the files of Advice
- Release of Accountant’s Advice for photocopying by the interested party
- Compile returned copy to folder

SECURING COPY OF FIRE SAFETY TAX STATEMENT OF COLLECTION
A Fire Safety Tax Statement of Collection is a list of taxes collected for the purpose within a quarter. This is used by the Bureau of Fire Protection Office as support to their monthly report to their Head Office and for any other purposes their office may have.
