SECURING COPY OF SERVICE RECORD

A Service Record is a document where you can find the record of government service of an employee.  This will be used for different purposes as a supporting document to any claims like loyalty pay and terminal leave benefits.  Other employees will also use this to support their loan application to GSIS and PAGIBIG.

FOLLOW THESE STEPS

IT WILL TAKE YOU

PLEASE APPROACH

  1. Register to the logbook Request for a Copy of Service Record.

  2. Personnel in-charge will print the service record.

  3. Submit to the LCE for signature (In case LCE is out of Town, the HRMO will certify the document)

  4. Release of Service Record

PROCESSING APPLICATION FOR LEAVE

A leave application is a civil service form filled out and filed by an employee during his/her leave of absence from the office.  This document is important in determining the total earned leave of an employee during his/her whole duration of government service.

FOLLOW THESE STEPS

IT WILL TAKE YOU

PLEASE APPROACH

  1. Register to the Log Book Application for Leave being applied.

  2. Submit to the Local Chief Executive for approval.

  3. HRMA/HRMO will deduct the approved numbers of days applied for leave to the Total Earned Leave of an Employee.

  4. Update the leave card of the employee.

  5. Compile to the 201 file employee approved leave.
